Safe system of work is structured developed procedure to carry out task safely ensure after identifying hazards and assessing risk. Its key elements include permit to work, risk assessment, hazards identification and implementation of control measures. As permit to work System (PTW) is part of SSW so it has very high importance in high risk industry such as oil and gas sector specifically, where safety is not just priority, but its necessity. A permit to work system is formal documented system or PTW is a formal written procedure that identifies and record the hazards associated with equipment, system, process, work and work area. This is also act as communication tool between Supervisor, Safety Officers, Worker and issuing authority such as area/asset owner. It clearly outlines:

Types of Permit to Work Systems

- Welding, cutting, open flames, Burning
- Use of spark producing tools, grinder, drill machines, Power hammer
- Non-explosion proof equipment such as photography.
- Hot Tapping, Non-destructive testing (NDT).
2. Cold Work Permit or General Work Permit
Cold work includes tasks or work in classified area or plant/well head that do not produce heat or sparks but may still be hazardous.
Example:
- Opening or disconnecting of and Isolated or closed pipeline or Vessel.
- Hydrostatic pressure testing.
- Flange opening.
- Scaffolding erecting or dismantling
- Civil Work(If no power tool need to be used)
- Painting.
- General Housekeeping in Classified area.
3. Confined Space Entry Permit
Confined Space is a space where a person can bodily enter, limited means of entry and exit and not designed for human occupancy. Confined spacy entry permit is required when workers need to enter enclosed or restricted spaces like tanks, vessels, or silos, pits deep than 1.2meter.
Note: Each permit is valid only for each confined space that is stated on the permit. This is permit is only for entry separate permit to work system is required for work to do in confined space.
Example:
- Doing a manual cleaning job in a vessel need and entry permit plus a cold work permit for the task or job.
- Gas cutting for removal of tray from vessel needs a hot work permit.

4. Electrical Work Permit
This permit to work system is used for any work involving electrical systems, especially high-voltage equipment.
Example:
- Repairing electrical panels in classified area
- Installation of electric distributor from electric/generator room to work site
5. Lifting Permit
Lifting operation shifting of heavy load from one place to another loading or loading of material by using crane, boom truck, forklift or powered lifting equipment requires a separate lifting permit for these operations.
Example:
- Removing of heavy equipment form location and loading on trailer.
- Offloading scaffolding material from truck by using forklift
Key Section of Permit to work System.
An Effective permit to work system has several critical sections each section has different attribute and importance which help to make full system effective, However format varies company to company how to compile there system we will discuss the section which most of organization follows.
Section A:
- Dates, Area and Unit Details.
- Equipment Number & details
- Job Description to be performed
- Tools and Equipment to be used
Section B:
This section is very important need to be fill very carefully by a competent permit receiver and checked by the permit issuer at time of issue ensure that no hazards and its controls missed
- Hazards Identification (All check related to above job description)
- Identification of Required Precautions (Include supporting Documents & permit)
- Equipment or Area Preparation (Barricading Tags, Fire Extinguisher Availability)
- Personal Protective Equipment (selection on the bases of job scope)
- Communication tools going to be used
Section C:
- Initial Gas testing in case of hot work in the classified area or during the live plant for all type of activity.
- Initial Gas testing done by the authorized gas tester before the start and issue of the activity
Section D
This section is for issuing authority:
- Issuance, Acceptance, Authorization
- Permit issuer details, and signature
Section E:
- Permit close out
- Work status
- Complete/Incomplete
Section F:
This section is for periodic gas test record on site during the activity. After complete period authorized gas tester again do the gas test and keep record of gas test on the permit in this section.
Section G.
- Extension of permit(normally for next shift if work is not ended in the present shift)
- Revalidation If any person change in authority or work stop due to any emergency
- Also final remarks and closed out of permit for record
